Dynamic Neuromuscular Stabilization Course "B"

The “Prague School of Rehabilitation and Manual Medicine” was established by keyneurologists/physiatrists, all of whom were giants in the 20th century rehabilitation era i.e. Karel Lewit and the late Professors Vaclav Vojta, Vladimir Janda & Frantisek Vele.  Based on groundbreaking neurodevelopmental and rehabilitation principles by these men, Professor Pavel Kolar has successfully integrated the work of his predecessors in proposing the underlying neurodevelopmental mechanism for how the movement system develops hand-in-hand with CNS maturation.  This complex approach is “cutting-edge” in that it provides a window into provides a window into the complexity and plasticity of the CNS and its effect on the movement system.  The DNS approach can be used in the rehabilitation of a myriad of neurologic, musculoskeletal pain syndromes as well as performance athletic training.

Dr. Frank received her physical therapy degree from Northern Illinois University. She completed the Kaiser Permanente Orthopedic Residency program in 1993 while working on her Master of Science degree in Physical Therapy at University of Southern California. She received her post-professional doctorate degree from Western University of Health Sciences, Pomona, California 2004. She is a board certified specialist in Orthopedic Physical Therapy and a fellow in the American Academy of Orthopedic Manual Physical Therapy. Her clinical career has been greatly influenced by Shirley Sahrmann PT, PhD, and the Prague School of Manual Medicine faculty, namely, the late Vladimir Janda MD, Karel Lewit MD, and Pavel Kolar PT, PhD.

Dr. Frank practices at a private clinic in Los Angeles, California. She has been instrumental in setting up the Movement Science Fellowship at Kaiser Permanente, Los Angeles. She has served on the medical team for the 2009 World Figure Skating Championships in Los Angeles, as well as the injury prevention team for the Chinese Olympic Teams 2010/11. She currently teaches in the U.S. and internationally and has co-authored “Assessment and Treatment of Muscle Imbalances: The Janda Approach”

BSMPG 2012 Summer Seminar Review by Bangen Athletic Development

This past weekend I had the pleasure of heading to Boston for my third Boston Sports Medicine and Performance Group (BSMPG) Summer Seminar. I wasn’t sure how it was going to top last year’s line-up and I can’t say for sure that it did, but it was at least on par. I was able to hear some very informative presentations from Bill Knowles, Craig Liebenson, Sean Skahan, Dan Boothby, Pete Friesen, Boo Schexnayder, Joel Jamieson, Cal Dietz, Chris Powers and Alan Grodin. The following are the three things that I am going to implement into my program at Michigan Tech as soon as I get back to Houghton tomorrow (I am writing this on the plane trip home).

1. Subjective Athlete Monitoring

Joel Jamieson spoke about allostasis and the training process. According to Joel, Allostasis is “the varying integrated adaptive responses taken by the body in order to maintain homeostasis at all times and in all circumstances as necessary to keep the body alive.” We only have a certain amount of energy (called the allostatic reserve) to accomplish these adaptations. Training is obviously one of the stressors that can disrupt homeostasis, causing allostasis to occur. However, if the training is too much in terms of volume, intensity, or both, then that allostatic reserve can be drained and we won’t get the training adaptations that we are looking for.

Therefore, it is essential to monitor the recovery levels of our athletes and quantify their training load to maintain that allostatic reserve and ensure readiness for training. Unfortunately, at Michigan Tech we don’t have the resources for an Omega Wave or even other heart rate variability technology. I also don’t have the time to measure everyone’s vertical jump or grip strength on a regular basis. However we can use subjective measures to tell how an athlete is feeling and how they perceive their training. We will now have the athletes rate on a scale of one to ten on how ready they feel to train that day after the warm-up (it’s best to do it after the warm-up because athletes rarely feel ready before, sometimes I don’t feel fully ready until after the warm-up) and they will rate their perceived difficulty and intensity after the training session. This will allow me to quantify how they are responding to training and make adjustments as needed.

2. Lowering Hurdle Height for Plyometrics

In our second, third and fourth phases of plyometric training we use hurdles to jump over and in the past we have allowed our athletes to bring their knees up, sometimes all the way to their chest in order to clear the hurdle. During Boo Schexnayder’s presentation, he repeatedly discussed the importance of posture while jumping and that the set should be stopped if posture is lost at any point.

Postural Function of the Diaphragm in Persons With and Without Chronic Low Back Pain

Despite the high prevalence of low back pain in the population, options regarding effective treatment strategies are still limited, possibly due to the lack of knowledge of the underlying mechanisms.9 Trunk stabilization and postural trunk control may play an important role in the etiology of low back pain.9 In turn, the function of the diaphragm may affect how the trunk is stabilized, especially during postural activity.11,15,22 Various studies have shown that the pelvic girdle and lumbar spine are reflexively stabilized and braced prior to the initiation of extremity movements.3,13,17,22 The central nervous system must be able to anticipate movement and stabilize the entire core musculature automatically to provide a stable base from which the muscles performing the movement can pull.

Trunk bracing maintains all spinal segments in a biomechanically neutral position during the course of any movement. Segmental movement (eg, hip joint movement) is therefore related to the synergistic activity of the spinal extensors and all the muscles modulating intra-abdominal pressure (ie, abdominal muscles, the diaphragm, and the pelvic floor). The diaphragm is the muscle that contributes the intra-abdominal pressure modulation and plays an important role in spinal stability.7,16,18,27,31

Insufficient function and poor coordination of postural, or stabilizing, muscles are considered to be important etiological factors in spinal disorders associated with low back pain, such as deformational spondyloarthrosis (with or without spinal disc herniation), spinal disc protrusion, and/or spondylolisthesis.5,12,21,25

CONCLUSION

We found reduced diaphragm movement when isometric flexion against resistance of the upper or lower extremities was applied.  The combined, more cranial position in the anterior and middle portions of the diaphragm and, particularly, the steeper slope between the middle and crural portions of the diaphragm in patients with chronic low back pain may contribute to low back pain symptoms. However, given that the results are based on crosssectional analysis, we cannot exclude the possibility of reverse causation. Still, the results support the theory that patients with low back pain complaints present with compromised diaphragm function,
which may play an important role in postural stability.


KEY POINTS

FINDINGS: We found reduced diaphragm movement in patients with chronic low back pain compared to healthy controls when isometric flexion against resistance of the upper or lower extremity was applied, mainly in the anterior and middle portions. This pattern of diaphragmatic recruitment resulted in a steeper angle in the middle-posterior part of the diaphragm and likely a greater strain during activity on the ventral region of the spinal column.
